Insurance & Fees

My fee is $105 per 55-min session. I will however, work on a sliding scale for those who due to financial difficulties are unable to pay my usual fee. I also take MasterCard and Visa for your convenience. I currently accept Blue Shield, Cigna, Tricare West, Western Health Advantage, Magellan and some EAP programs. I am also happy to bill any insurance company as an out-of-network provider if you have a PPO plan.

I also accept Health Savings Account cards, Visa, MasterCard, Discover and American Express.

Availability

I am available for appointments Monday-Friday between the hours of 10:00am - 6:00pm . I do not work weekends.

Forms

Once we have scheduled an appointment, please download and complete/sign the following Intake Form and Hippa Privacy Policy and bring them with you to our first appointment. I cannot see you without these completed forms, so if you forget to bring them, I can give you a copy to complete in the waiting room before we start the session; however, it will save us a lot of valuable session time if you have them prepared beforehand.
